Dena Sports Physio protects the personal information provided by patients and website visitors.
Information we collect
We collect only the information you choose to send through our appointment form: your name, phone number, email address, preferred clinic and type of appointment, whether the visit relates to a CNESST or SAAQ file, your preferred days or times and, if you wish, the reason for your visit in a few words.
How we use it
We use this information to respond to your request, schedule care, and communicate with you. Please do not submit detailed or sensitive medical information through the website form.
Service providers
The form is sent to our website's server and delivered by email to the clinic's mailbox. Both are operated by our web hosting provider (HostGator), whose servers may be located outside Quebec; they are used solely to transmit your request. Access is limited to people who need the information to respond to you. Pages that show our clinic locations include an embedded Google Maps map; when it loads, Google may collect technical information (such as your IP address and browser details) under its own privacy policy.
Visit statistics
Visit statistics (Google Analytics, which uses cookies) are collected only after you accept them in the banner shown on your first visit; you can decline, and you can change your choice at any time with the button below.
How long we keep it
Requests sent through the website are kept only as long as needed to answer them and schedule your care. Information that becomes part of your clinical record is kept as the rules of the professional orders require.

You may ask about, access or correct your personal information, or withdraw your consent, by writing to info@denaphysio.ca. The person in charge of the protection of personal information at Dena Sports Physio can be reached at the same address. If you are not satisfied with our answer, you may file a complaint with the Commission d'accès à l'information du Québec.
